Basic Life Insurance and AD&D
As a full time employee, Brooks provides Basic Life and Accidental Death & Dismemberment (AD&D) insurance at no cost to you.
Employees receive a generous benefit of 1.5x your annual compensation through MetLife.
During the enrollment process, be sure to designate a beneficiary.
Voluntary Supplemental Life Insurance
Voluntary Supplemental Life Insurance is available to you and your covered dependents, on an optional basis, through MetLife. You must elect Voluntary Life Insurance for yourself in order to elect either your spouse and/or child(ren) voluntary life insurance coverage.
Coverage is Available:
For you
1-3x your annual compensation, guaranteed as a new hire up to $250k.
Maximum coverage $750,000.
For your spouse
Coverage may not exceed 50% of your combined employee basic and supplemental coverage, up to $30k guarantee.
Maximum coverage $100,000.
For your child(ren)
Coverage amounts available in increments of $2,000 up to $10,000.
Guaranteed issue means regardless of your health, you cannot be turned down or declined for this coverage.
After your initial offer of coverage, any increase or new election for yourself or spouse will require a medical questionnaire to be submitted to MetLife. This medical questionnaire can be accessed through the benefits administration system, benefits.plansource.com.

Rates shown are per pay period for 24 pay periods (you will have 2 pay periods without a contribution).
