Disability Plans

Short-term and long-term disability insurance coverage is available to you as a full-time employee of Brooks Rehab.

Short-term disability (STD) is available for full-time employees to purchase either a Base or a Buy-Up option. This benefit begins on the 15th day of a qualified disability and pays for up to 11 weeks.

Basic long-term disability (LTD) is offered to all full-time employees at no cost. Eligible employees have the option to add an LTD buy-up plan.

Short-term Disability

Short-term disability (STD) is available for full-time employees to purchase either a Base or a Buy-Up option. This benefit begins on the 15th day of a qualified disability and pays for up to 11 weeks.

Long-term Disability

Basic long-term disability (LTD) is offered to all full-time employees at no cost. Eligible employees have the option to add an LTD buy-up plan.

MetLife gives new hires the opportunity to enroll in short-term disability or long-term disability buy-up plans on a guaranteed issue basis without answering any medical questions.

Guaranteed issue means regardless of your health, you cannot be turned down or declined for this coverage.

Short-Term Disability (STD) Plans

STD Base Option
BenefitIf approved after 2 week waiting period, you will receive:
  • 60% of your weekly base earnings
  • To a maximum weekly payment of $500
STD Buy-Up Option
BenefitYou can elect to "buy-up" your STD to receive higher weekly payments:
  • 66.67% of your weekly base earnings
  • To a maximum weekly payment of $1,500

Maternity leave for full-time employees would be covered under the elected STD benefit and would pay at least four weeks for a vaginal delivery and six weeks for a C-section delivery.

Long-Term Disability (LTD) Plans

LTD Base Option
Benefit - Employer PaidIf claim is approved, you will receive after 180 days of disability:
  • 50% of your monthly base earnings, to a max of $12,500 per month
LTD Buy-Up Option
BenefitYou can elect to "buy-up" your LTD to receive:
  • 10% more or total of 60% of your monthly base earnings

If you have received treatment for any condition three months prior to the effective date, you will need to be treatment-free for 12 months from your effective date before benefits will be paid for the pre-existing condition.

Rates shown are per pay period for 24 pay periods (you will have 2 pay periods without a contribution).

Base Option$0.88 monthly per $10 weekly benefit
Example: Example: Susie makes $700 per week. The weekly disability benefit would be 60% of that, or $420. $420/$10 x $0.88 = $36.96 per month.
Buy-Up OptionAdditional $0.27 per $10 of weekly benefit on top of the cost for the base plan
Long-Term Disability (LTD) Rates
Buy-Up$0.142 per $100 of monthly income
Example: Bob makes $35,000/year or $2,917/month. $2,917/$100 x $0.142 = $4.14 per month *The Base Option is paid 100% by Brooks.
