2022 Brooks Rehabilitation Benefits Guide

This booklet is an overview of the comprehensive benefits package offered to you by Brooks.

What's New for 2022

Medical Plans

This year, in addition to the HSA 1500 plan, Brooks will also be offering the PPO 1500.

Although this plan is not compatible with the Health Savings Account, it offers copays for office visits, urgent care center services and prescriptions.

Health Savings Accounts

If you enroll in the HSA 1500 plan, eligible members will also be enrolled in a Health Savings Account (HSA). For 2022, our new HSA administrator will be Accrue Health.

Accrue Health is BCBS’s preferred vendor and will allow you to view your HSA balance through your BCBS myhealthtoolkitfl login.  You will be receiving a new HSA card mid-December and funds from your HSABank account will be transferred early 2022.

Short Term Disability

Brooks will be providing all full-time employees with an employer-paid Short Term Disability plan. You will also have the opportunity to buy-up for additional disability coverage.

Long Term Disability

You will have the option to enroll in a Core or Enhanced Long-Term Disability plan for 2022. Both options will be employee-paid and will be offered this year without having to answer any medical questions.
